Medical Society Supports Principles of the Patient Centered Medical Home
The patient centered medical home (PCMH)—which encourages comprehensive primary care through a coordinated, physician-led health care team—has earned the support of the Pennsylvania Medical Society.
The Society’s Board of Trustees in February 2008 adopted an amended resolution based on a resolution from the Society’s 2007 House of Delegates urging support of the principles of PCMH.
This chronic disease management model was developed and supported by the American Academy of Pediatrics, the American Academy of Family Physicians, the American College of Physicians, and the American Osteopathic Association. The model emphasizes:
- A personal physician for each patient
- Physician-directed medical practice
- Whole person orientation
- Coordinated/integrated care
- Quality and safety
- Enhanced access to care
- Payment structures that support PCMH
The Society supports the implementation of demonstration projects of this model of health care delivery and will work with stakeholders to integrate Medicare and Medicaid patients into the projects.
