Specify the following about BCIS:

• Symptoms
• Pathophysiology
• Risk factors
• Risk reduction strategies

Article 2: Confirming Feeding Tube Placement: Old Habits Die Hard

• Recognize that traditional feeding tube placement verification methods may not be as reliable as current, evidence-based practices for verification
• Identify general patient risk factors for potential tube misplacement.
• Identify risk reduction strategies that may reduce the risk of tube misplacement.

Article 3: Let’s Stop the Bleeding: Preventing Errors with Heparin Therapy 

• Identify potential errors involving heparin and propose recommendations to lessen their impact.
• Review specific causes of medication errors with the use of heparin and propose recommendations to avoid these causes.
• Identify potential errors with heparin products due to look-alike labeling and propose recommendations to lessen their impact.
